Non-Insured Fees
The following services are not covered under insurance and are subject to fees.
- Sick Note (Work/School):$25.25
- Travel Cancellation Insurance Form:$160.00
- Letter (Massage, Orthotics, Back Support, Stockings):$30.00
Physician Hourly Rate for Legal Consultations and Non-OHIP Services
- Dr. Frederick Ma: $950 / hour
- Dr. Ognjen Visnjevac: $950 / hour
Medical Records Requests
Fees for copies of medical records are applied as follows:
- $30.00 for the first 20 pages
- $0.25 per additional page
- Additional charges may apply for postage, courier, or storage retrieval
- Physician review (if required) is billed at the hourly rate
These fees follow standard medical guidelines and cover administrative and processing costs.
Cancellation & No-Show Policy
At Bloor Pain Specialists (BPS), we are committed to providing timely and high-quality care to all patients.
To ensure efficiency and minimize wait times, we maintain a strict cancellation and no-show policy.
If you are unable to attend your appointment, please notify us at least
24 hours in advance (excluding weekends and holidays).
Definitions
- Late Cancellation: Cancelled less than 24 business hours before appointment
- No-Show: Failure to arrive within 10 minutes of scheduled time without notice
Missed Appointment Fees

Important: Fees must be paid before booking a new appointment.
Emergency cancellations (such as hospitalization or severe illness) may be exempt from fees,
